1. Patients are responsible for allowable balances that the insurance company does not pay.
2. Many insurance companies do not cover the refraction charge, which is due at the time of service.
3. All co-pays are due at the time of service.
5. A minimum of 50% payment is due in order for us to start your glasses order. Final payment is due when items are picked up.
6. Please allow 24 hours notice when changing or cancelling your appointment.
7. New patients or patients with new insurances should arrive at least 20 minutes in advance of your appointment time.
